FYHP - Coaching Evaluation

Dear Hockey Families,

The success of our program and the development of your player are closely tied to the quality of our coaching. To ensure continued improvement, it is critical that we gather feedback to help our coaches make any necessary adjustments.

The Board of FYHP kindly requests that parents complete an evaluation for each of their coaches. Please be frank and honest in your responses—your input is essential to improving the quality of coaching for next year.

If your player has multiple coaches, we ask that you submit a separate survey for each individual coach or focus on evaluating the head coach.

We would greatly appreciate receiving these evaluations based on the coaches’ performance this season.
